The deadline for the registration is Friday, November 21, 2014.　Registration is closed.

Please complete both KEK User registration and Workshop registration as follows. It will take a few days for KEK user registration to complete, and you can proceed to the workshop registration only after that. So, please register early.

Please note that we may not be able to provide support for all who applied. If you have your own funding, we ask you to use it. Also note that we cannot cover airfare from foreign countries. Only domestic travel and local expenses can be considered.

Step 1. KEK User registration

1. If you have already had your User ID and Password, please go to Step 1'.
2. Visit the webpage "KEK User Information" at http://uskek.kek.jp/eng/
3. Click "+User Registration" in the left menu
4. Click "3. User Registration" just below.
5. Read the terms and click "Agree" at the bottom.
6. Fill in the blank using the following information:

7. After "Continue", please confirm the next page and push "Agree".
8. You will receive your User ID and Password by e-mail within 2 business days (via human processing).
9. Change your Password within 48 hours, otherwise it will be expired.

Step 1'. KEK User registration

1. This Step 1' is only for those who have already got their User ID and Password on the previous visit to KEK in 2007 or later.
2. Please login at the webpage: https://krs.kek.jp/uskek/ui/UI_00000E.do
   (User ID is your registered e-mail address.
   If you don't remember your Password, please e-mail Users Office <uo1 (at) mail.kek.jp>.)
3. Click "+Administrative Procedures for Visiting KEK/J-PARC" in the left menu.
4. Click "2. Identify the purpose of your visit to KEK/J-PARC" just below.
5. Fill in the blank using the information given in Step 1 above.
6. Click "Next" and "Agree".
7. You will get e-mail within 2 business days (via human processing).

Step 2. Workshop registration

After receiving your User ID and Password, please visit https://krs.kek.jp/uskek/ui/UI_00000E.do
and login with your User ID and Password.
(If you don't remember your Password, please ask Users Office for a temporary Password.
When you receive the temporary Password, please change it in 48 hours or it will be expired. )

1. Click Administrative Procedures for Visiting KEK/J-PARC
   =>1. Administration relating to your visit
   in the left panel.
   
2. Push [Next] button at the item:
   Project No.: KEK Theory Workshop 2015
   
3. Please push [Submit] button of the item "Registration for gathering".
   Fill out "KEK Theory Workshop 2015 Registration form" and [Submit] at the bottom.
   
   * To invited speakers: Please choose Oral presentations, and submit title/(abstract).
   * To apply for a short talk: Please choose Oral presentation and submit title/abstract.
   * To apply for a poster presentation: Please choose Poster presentation and submit title/abstract.
   * Banquet will be held in the eveing on 29th Januraty.
   * If you are requesting supports for local expences, note so in the "Remarks" box and send an e-mail to our secretary, Ms. Tamao Shishido.
   
4. Please [Submit] "Application for Wireless LAN access (Tsukuba Campus)" form if you would like to have the Internet connection.For instructions on finding MAC address of your computer, see http://uskek.kek.jp/eng/other/lan2.html

Step 3. Bank account registration for reimbursement

Only for people who ask reimbursement through Japanese bank account

1. Please login at the webpage: https://krs.kek.jp/uskek/ui/UI_00000E.do
2. Click "+Administrative Procedures for Visiting KEK/J-PARC" in the left menu.
3. Click "1. Administration relating to your visit" just below.
4. Click the "Next" button.
5. Click "Submit" for "Request for bank account number registration & change (Japan only)".
6. Click "Search" of Financial Name.
7. Enter Branch name only and click "Search".
8. Select Bank name at the left circle and click "Confirm".
9. Fill the form and click "Next" at the bottom.
10. Check the information and click "Confirm" at the bottom.

Step 4. KEK Dormitory

We recommend early booking since many guests visit KEK for the period.
You can book a room of KEK Dormitory from 45 days in advance.
